You might try Ashville Drivetrain in Ashville, NC. No personal dealings with them but I know people that have had them build shafts and they do a stand up job. I'll be needing one for a Ford project in a couple months and I plan to go with them.

I've heard good things about Atlanta Driveline. I haven't personally used them tho.
