6.4 factory water separator problem?

Buddy's truck, haven't actually seen it myself.

Said when he drained the water separator today, it drained about 2 cups of water, and then some chunks of "something" starting coming out in the water, and then a tiny bit of diesel started coming out in the water.

I told him all I could think of was maybe there's an actual filter in there, like a cummins setup, or an aftermarket lift pump setup, and the filter was starting to fall apart and come out with the water.

What should I actually tell him? Thanks.
