Surging, hesitation under acceleration.

Rolling into 35-40% throttle from a stop. The truck hesitates, and rpm's surge back and forth from 1700-2000. Data logging shows rail pressure is exactly where it should be. I don't think it would be a fueling issue. Rolled into the throttle at 45mph (O/D off) to 65mph at 70% throttle. Letting off the throttle, the truck dies. Shifted into neutral and started immediately.

(Fuel - 200gph LP, 250% injectors, stock &12mm cp3's) all new. New engine, ported head, 188/220 cam, 15.3 compression pistons, 76mm turbo.

Would having the compression lower cause this? Small pin hole in the intercooler, could a charged air leak cause this? The only difference I've seen with tuning is with the Main injection timing.
 
Was thinking a combination of charged air leak and possibly a bad MAP sensor. There is hardly any smoke from exhaust.
