So you just got it built and then you had to go back and rebuild it yourself?

So what was the problem. I see the spigot bearing hanging out the back of the fly wheel. Was the input shaft not true to the centre line of the crank.

No got it all built at once new 1 3/8 input ,new updated main shaft ,bearings ,seals and syncros

The people who put the last clucth in didn't even pull the flywheel to take the old pilot bushing out they sttacked them in the flywheel

So is this the end-cause of the cracked T-case bearing housing? Makes more sense than the front drive shaft.
I'd be talkin' to someone at the rebuild shop.

No I bought the truck early this year with 5th gear housin cracked I dropped the tcase off and replaced it with the cast one. Then put everything back together drove it for a few days with no problem. I then loaded my cr on my trailer and hauled it to a pull 200miles away from home. Trans was seapin oil out the top where someone tried pullin the top cover with out pullin the trans (they cut the floor of the cab also) 3rd gear would grind also. On the way home I was also haulin a 1500lb lathe with my trailer too when I cracked the t case. The front driveshaft is to long that's why the 5th gear housin cracked the first time (which when I removed it also saw where some one tried fixin it before by weldin it) I just found the other weak point the tail housin. A friend of mine truck did the same thing but his shaft locked up in the extended position broke the same t case parts.
