Melted pistons

Curious if anyone has seen a truck melt 5 out of 6 pistons in one shot. We dyno'd a friends truck yesterday and went we pulled it apart, thats what we found.

06, 5.9 120's, sb 66, ppe dual cp3's, smarty TNT-R. Four runs on lvl 4 on smarty. 1 run on lvl 5. Had cool down time between runs. All gauges looked normal except fuel pressure was low. We figure it didn't get enough cool down between runs, and dyno operator lack of experience. The last run when it blew it was loaded pretty hard. Took a long time for rpm's to climb. Basically it got hot really fast. Didn't realize that it would melt that quick. However, now we know.........
 
Mp-8 but it was off, never got that far. Basically we are pinpointing it to be heat soaked cooling system, too much dyno load( setting up the dyno)
 
On some runs the fuel gauge was showing 0psi and you could hear the truck dying out. Were going to get the injectors tested and see what that brings back for information. Still can't believe how fast it melted
 
Also, if you had low fuel pressure wouldn't that also impact your rail pressure, reducing it as well?
