What causes injector failure

Like the title says what causes these failures. My dad has a late 04 and this is his 4th set in 210000 miles. Is it a lack of fuel issue or lack of filtration or what?

Usually lack of fuel pressure, or air in the injectors. 210K wow, thats good to hear.

Low fuel pressure
bad fuel...or weird fuel
no lubrication
bad filters/maintenence
and then some just go bad for the fun of it. Those spool valves suck.

What are some other problems you have had in your 200,000+ miles of ownership?

This is what I can remember of the top of my head.
2 turbo
1 set head gaskets
1 cps
1 ipr
4 sets of injectors
and countless egr problems
The truck gets worked hard 2-3 days a week and the rest are highway miles.
